Are low and high utilization related to the way GPs manage their practices? An observational study

General practice care plays a key role in keeping healthcare effective and cost-efficient. However, variation in the utilization rates of practices may reveal variation in practice performance. This research goal is to investigate whether the socio-demographic profile of the patient´s area of residence and practice organization characteristics influence the low or high utilization of general practice care.

ValueCare: Cork/Kerry Pilot Site

In Ireland, the ValueCare project focuses on co-designing a digital solution to help older people (≥75) experiencing mild to moderate frailty to live independently in their homes. The technology will foster regular communication between older people in the relevant organisations (primary, secondary and social care) with technology enabled data sharing & common data sets can improve flow and information exchange. Older people will access the VALUECARE integrated care pathway via referral from primary care, the emergency department, the acute medical unit or the public health nurse. The care coordinator will develop the following 4 components; case finding; needs assessment; care planning and care co-ordination.

A Cross-European Study of Informal Carers’ Needs in the Context of Caring for Older People, and their Experiences with Professionals Working in Integrated Care Settings

Informal carers are increasingly relied on for support by older people and the health and social care systems that serve them. It is therefore important that health and social care professionals are knowledgeable about and responsive to informal carers’ needs.

This study explores informal carers’ own needs within the context of caregiving; and examines, from the informal carers’ perspective, the extent to which professionals assess, understand and are responsive to informal carers’ needs.
